Network primary care clinic in Centralia is closing

Cascade Family Medical Clinic, a network primary care clinic in Centralia, will be permanently closing its practice on Friday, August 30, 2019. The clinic’s providers include Eric Demun, MD; Christopher Yarter, MD; and Stephen R. Fischer, PA. Kaiser Permanente members who receive care at the clinic will need to find another primary care provider and have their medical records transferred before August 30.

Helping members transition their care

The clinic notified its patients of the upcoming closure in late May. We sent a letter to our affected members last week, telling them about their options and offering assistance in transitioning their care.

We are directing members to other in-network primary care providers in the Centralia and Lewis County area, and to Kaiser Permanente Olympia Medical Center, where they can get one-stop medical care and fill prescriptions. Another option for care will be our new Kaiser Permanente medical center in West Olympia, which is scheduled to open in December 2019.

HMO and Medicare members who have not selected a new provider by September 29, 2019, will be auto-assigned a new provider. PPO members will not be auto-assigned to a community network provider unless they contact Member Services.
